A previously approved action could execute a different executable than the operator approved.&lt;/p&gt;

Node `system.run` approvals now require immutable `systemRunPlan` data, and path-token commands are pinned to canonical executable identity (`realpath`) across approval and execution.&lt;/p&gt;
